If you are requesting records – prescriptions, exam histories, or copies of your invoices – you will be provided instructions on how to access your records online through the Patient Portal.
If you are requesting records to be sent to another provider, please provide:
Records will be provided directly to the requested provider within 48 hours.
If you are in need of quicker response time, please log into your Patient Portal and download your records free of charge 24/7 or pick up a printed copy during our normal business hours.
